The Behavioral Health Home at Dirigo Counseling Clinic
The Behavioral Health Home is a service model that reflects a more modern approach to traditional case management services. This is available to Medicaid clients, with some exceptions, who also have two or more chronic conditions such as asthma or diabetes.
The Behavioral Health Home aspires to more intensive care with the “triple aim” of:
- Better outcomes
- Higher patient satisfaction
- Lower costs
The notion of “Home” means that all of your health services are more integrated and centralized rather than fragmented. Under this care model, there is more collaboration among providers and a central Plan of Care to assure all of your health needs are considered in one central Plan.
